2. I confirm that the information I have provided Opunake Hydro is
correct and certify that I am authorised to initiate the
switch/transfer to Opunake Hydro. I understand that Opunake Hydro is
under no obligation to accept any person as a customer.
4. I authorise my current electricity supplier and distributor
to disclose to Opunake Hydro (and its authorised subcontractor) any
metering, payment history, billing and usage information relating to me
(including but not limited to in respect of the above premises) and to
cease supplying and delivering me electricity.
5. I appoint Opunake Hydro to act as my agent in respect to all
matters relating to the supply and delivery of electricity to the above
premises. This includes finalisation of my account with my current
electricity supplier on the basis of meter readings that may be
estimated. Should the switch from my current supplier fail for any
reason, Opunake Hydro has the right to cancel the switch/transfer.
6. I authorise Opunake Hydro to disclose relevant information
enabling me to be identified to its credit checking agency for the
purpose of Opunake Hydro obtaining a credit check about me and/or
authorise Opunake Hydro and/or its credit checking agency to contact
any referees nominated by me.
7. I understand that Opunake Hydro will hold my personal
information for the purpose of supplying me with electricity and
associated services. From time to time my contact details may be used
by Opunake Hydro to provide me with offers and information from Opunake
Hydro and other approved agencies.
8. I will inform Opunake Hydro if I do not wish to receive
these offers and information.
9. I understand that I have the right to access and correct
information held by Opunake Hydro.
10. I acknowledge and agree that all equipment supplied to my
premises by Opunake Hydro remains the sole and exclusive property of
Opunake Hydro and does not become a fixture to my premises.
11. I understand that Opunake Hydro funds the basic Smart Meter
installation at my premises. I acknowledge and agree that this is
funded on the basis that I remain a Opunake Hydro customer and that if
I cease to be a customer within 3 years, I may be liable to pay Opunake
Hydro fees associated with disconnection and removal of the Smart
Meters to the amount of $50 + GST for every year and/or part year that
is remaining; in addition to an administration fee of $85 + GST.
