Maori Health Team
Tui Maxwell - Outreach Nurse
Tui’s nursing role is to work with the Health Centres in the Wellington region, receiving nurse/doctor referrals to follow up our enrolled Maori whanau who are overdue for:
Cardiovascular risk assessments (heart checks)
Diabetes annual reviews and testing
Immunisations for tamariki and kaumatua
Tui also assists whanau in an additional support/advocacy role to ensure Maori whanau are able to maintain a healthy lifestyle by:
Providing transport to your medical appointments at the medical centre or hospital
Networking with you and other organizations WINZ, Housing NZ some social support services
As an outreach mobile service Tui is able to meet with you where you are most comfortable.
Services for Maori
Outreach nurses help people who have difficulty accessing their GP or practice nurse for reasons such as cost, transport, language, and/or cultural barriers. Outreach nurses can provide assistance for GP visits, transport to appointments and medications. They also work with other agencies such as Work and Income and CYFS to ensure other social needs are being met.
Our Outreach Nurses are a mobile service, and they can meet with you where you are most comfortable to discuss your health issues. They can help you with:
