Older Adults Community Support

Sustainable Solutions – Healthy Living

Older Adults Community Support Service is an intensive and flexible mobile support service for adults aged 65 years and over who experience mental illness. The service is a wrap around one for individuals who have multiple needs. Hours of support can be adjusted from week to week. We support our clients spiritually, mentally and physically and offer a variety of services to suit their individual needs.

Assistance is given to help individuals manage their wellbeing as well as:
• Assistance to remain in current housing
• Support to transition to different living arrangements
• Widen friendship networks
• Liaise with other services providing support to the individual
• Involve family or friends in the wellbeing journey
• The opportunity to receive support from Equip’s Spirituality Co-ordinator

Support Workers use a people centred approach. We use the tools of Professor Charles Rapp's Strengths Model, in order to attain self-directed goals and dreams.

Support Workers assist individuals to identify their strengths and maximise their potential in their community. The Support Worker works with individuals to access essential resources within their community to increase their opportunities and choices.

Support Workers work closely with an individual's clinical key worker and / or whānau family where appropriate. Recovery, hope and an improved quality of life are more likely to occur when services and people who use them, work together.

How to access this service

Anyone over the age of 65 who has a mental health diagnosis, such as
• Schizophrenia;
• Bipolar disorder;
• Depression; or
• Anxiety disorders

People can access this service via the Co-ordination Service at the Waitemata and Auckland District Health Board’s Older Adults services.

People can access this service provided they have a current clinical key worker or GP and meet minimum criteria. Support Workers assist individuals to identify their strengths and maximise their potential in their community.
